All students must complete an insurance mandated registration form prior to participation. Registration forms updated each September. Please:

1) Download the registration PDF file below.

2) Please read (call studio if you have questions.)

3) Fill in all blanks (please write clearly.)

4) Sign and return to the studio during fall registration or the first day of class participation.
